About the Role:At Saint Vincent Hospital, we are seeking a skilled Social Worker to join our team in a per diem day position. This individual will play a critical role in facilitating care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources.Key Responsibilities:

• Conduct complex psycho-social assessments and interventions to promote timely throughput, safe discharge, and prevent avoidable readmissions.
• Integrate national standards for case management scope of services, including transition management, care coordination, and compliance with state and federal regulatory requirements.
• Lead a population of patients by service line and/or lead the team by being a resource to Tenet performance standards.Requirements:
  • Master's of Social Work degree
  • 2 years of acute hospital experience preferred
