Job Summary: BAYADA Home Health Care is seeking an experienced Weekend Physical Therapist to join our team. As a skilled Physical Therapist, you will make home care visits to patients in Senior Living Communities, providing personalized care and rehabilitation services.

Responsibilities:

• Conduct thorough assessments of clients' physical abilities and develop individualized care plans.
• Implement manual therapeutic exercises, gait training, balance, and other interventions to promote clients' health and safety.
• Collaborate with nursing staff to revise and update care plans as needed.
• Educate clients and their families on rehabilitative care and activities necessary for independent living.
• Accurately document observations, interventions, and evaluations related to client care management and services provided.
  Qualifications:
  • Current licensure as a Physical Therapist in Massachusetts.
  • Minimum of one year of recent clinical experience as a licensed Physical Therapist.
  • Ability to travel to assigned cases.
